Please complete this form ONLY if you have already talked to Anita and she has stated we have space and can receive your cat(s). This form is for cats originally adopted from Spay the Strays who are being returned and for those who wish to surrender animals they've found or can no longer care for. Please click the yellow PayPal donate button to send a donation via PayPal, debit or credit card as this surrender will add costs to us and we are NOT government funded. (Our PayPal is SpaytheStrays@Aol.com) Have you talked to Anita and been given the approval to surrender?
Please list the name of the pet(s) to be surrendered. (If it's a return, please include original pet's name and foster, if you know it).*
Please provide approximate age, gender, color and physical description of surrendered animal. Include if they are spay/neutered, declawed, microchipped, vaccinated, etc.*
Why are you surrendering the cat(s)?* Choose one: Allergies of a family member Behavioral issues Can't afford to take care of Death of original owner Doesn't get along with other pets Found cats (not mine) Litterbox issues Moving New baby Original owner going to assisted living/nursing home Some other reason
Please explain why you are surrendering in more detail so that we know what areas to address/correct:*
Please provide where you originally got the cat(s) from. We need name of organization if it was a shelter/rescue. (If not from a shelter/rescue, you can put stray found on streets, friend had kittens and I took one, etc.)*
Who is the name of your vet and when was your pet last seen/vaccinated?*
Please send a copy of all vet records to Marketing.SpayTheStrays@aol.com with subject line "Vet records for" and name of pet(s). You can also call your vet and have them send the records to the same email. Please confirm this has been done below.
Is the cat on any type of special food or have any special needs we should be aware of? Has the cat been diagnosed with ANY type of medical condition? Please list what food(s) you have been feeding currently (wet, dry, treats & brand names/types)*
The more you can tell us about the animal(s), the better we can place them. Please give us as much detail as possible.
If you're surrendering more than one cat, are they bonded (must be together or they're stressed out/love to be together all the time/anxiety without each other)? Are they okay together but don't necessarily have to be together? Are they pretty independent of each other where they're okay with other cats but don't have to be with other cats? (The more detail, the better we can match them to the right adopter)*
The cost of vet care, flea prevention and regular food and litter for cats can be very expensive. We are not government funded and rely on donations. The cat you surrender to us may not be adopted and someone will need to take care of it, potentially, for 15-20 years. Have you clicked the Yellow PayPal Donate button on this page to send a donation for the cat you have already been approved to surrender?
Once pick up is coordinated, the cat(s) will go into foster care, if a foster can be found. The cat will be advertised online via our website, partner websites and social media platforms. The cat(s) may also be put in our habitats at our pet store locations where volunteers visit once per day to ensure they have fresh water, food, litter, etc. To help us rehome the cat, please tell us about it's personality and any features you think may help "sell" them. (Please email any pictures to Marketing.SpayTheStrays@Aol.Com)*
Did you originally adopt the cat from Spay the Strays? If so please provide the location or describe as best you can.*
Please provide the microchip number(s) of the cat(s) you are requesting help with.*
Please provide the original name of the cat and foster, if you can recall either.*
Please provide last known date the cat(s) was flea treated. If it's been more than a month, please put NEEDS TO BE TREATED.*
